Bottom-Up Regulation: Rewiring the Nervous System Through the Body

Five body-first practices — from trauma-informed yoga to resonant breathing to EMDR — that recalibrate a dysregulated nervous system before cognition can.

1. Trauma-Informed Yoga (TCTSY)

Van der Kolk heavily features Trauma Center Trauma-Sensitive Yoga (TCTSY), a practice developed alongside David Emerson. Unlike conventional yoga classes, it focuses entirely on internal sensation rather than proper form or aesthetics.

  • Interoceptive Inquiry: Practitioners are invited to notice internal physical sensations in a non-judgmental way (e.g., "Notice if your shoulders feel tight or soft right now").
  • Choice-Making and Agency: Language is strictly invitational (e.g., "When you are ready, you might choose to extend your arm, or keep it rested"). This directly counteracts trauma's signature dynamic: loss of control over one's own body.
  • Grounding Postures: Simple, low-risk movement patterns (such as Gentle Cat/Cow or Mountain Pose) help anchor awareness in the feet and spine.

2. Heart Rate Variability (HRV) & Resonant Breathing

To rebuild parasympathetic tone ("rest and digest"), the book recommends specific breathing exercises designed to maximize Heart Rate Variability (HRV) — the natural fluctuation in time intervals between heartbeats.

  • Extended Exhalation (Coherent Breathing): Inhaling for 4 counts and exhaling for 6 counts (or a steady 5-in/5-out cadence at roughly 6 breaths per minute). Long exhalations stimulate the vagus nerve, signaling safety directly to the brainstem.
  • Diaphragmatic Awareness: Placing a hand on the abdomen to track physical expansion on the in-breath, helping reverse the shallow chest breathing common in hyperarousal.

3. Somatic Experiencing Concepts (Peter Levine's Work)

Van der Kolk draws on Peter Levine's somatic principles to help discharge stuck survival energy from fight, flight, or freeze states.

  • Pendulation: Alternating attention between an area of physical discomfort/contraction and a "neutral or safe resource" in the body (e.g., feeling tension in the chest, then shifting focus to the solid sensation of hands resting on the lap). This teaches the nervous system that discomfort is tolerable and dynamic, not permanent.
  • Titration: Processing bodily sensations in tiny, manageable increments to prevent overwhelming the nervous system or triggering a re-traumatizing flood of panic.

4. EMDR (Eye Movement Desensitization and Reprocessing)

While often facilitated by a clinician, EMDR functions as a body-centered protocol using bilateral stimulation:

  • Rhythmic Tracking: Moving eyes side-to-side, tapping alternating shoulders (butterfly hug), or listening to alternating audio tones while holding a disturbing image in mind.
  • Physiological Reprocessing: Bilateral movement mimics the physiological state of REM sleep, allowing the brain's thalamus to integrate raw, sensory trauma fragments into explicit memory without the accompanying fight-or-flight body surge.

5. Rhythmic Group Movement & Expressive Arts

The book highlights collective, rhythmic activities that re-engage the Social Engagement System (the ventral vagal network):

  • Communal Drumming & Chanting: Shared rhythm synchronizes breath, heart rates, and motor responses across individuals, restoring a visceral sense of belonging.
  • Theater & Movement Improv: Exercises requiring physical presence, voice projection, and mirroring another person's body language. These safely re-introduce physical boundaries, assertion, and spontaneous expression.
